Job Description
We are seeking a dedicated Family Practice Nurse Practitioner for a 48-day locum tenens assignment in South Dakota starting Oct 13, 2025. This flexible position offers 8-hour shifts across 1-5 days per week, focusing on functional medicine and advanced hormone therapy via telehealth consultations and follow-up care.Job Details

Responsibilities and Duties
- Conduct initial consultations and new patient lab reviews via video
- Review and interpret advanced labs including GI-MAP, Methyl Detox/Genomic Panels, ALCAT/food sensitivity testing, and comprehensive hormone/metabolic panels
- Develop personalized care plans encompassing lifestyle, nutrition, supplementation, and prescription therapy
- Manage asynchronous follow-up care, including secure messaging, protocol updates, and lab interpretation through EMR platforms
- Educate patients on test results and provide health optimization coaching
- Complete clinical documentation in EMR (CHARM/CANVAS) within 24 hours
- Deliver concise, functional medicine–focused SOAP notes and justifications for care plans
- Coordinate care with support staff, including RNs, MAs, and pharmacy teams
- Participate in chart audits or case reviews as requested by leadership
- Prescribe and manage bioidentical hormones, peptides, nutraceuticals, and other therapeutic agents as clinically appropriate
- Monitor patient responses, adjust dosing, and accurately document all medication changes
- Collaborate with compounding pharmacies and support teams for medication delivery
- Prescribe FDA-approved medications such as Metformin, Tadalafil, Testosterone, Progesterone, Estradiol, and Armour Thyroid
Additional Information
- Candidates with experience in functional medicine and hormone therapy are preferred; however, adaptable candidates with a foundational understanding of hormone therapy will be considered
- Must hold an active Nurse Practitioner license in South Dakota (or eligibility)
- Required board certifications: Adult-Gerontology Nurse Practitioner (A-GNP) or Family Nurse Practitioner Certification (FNP)
- DEA certification required
- Comfort with rapid learning and varied backgrounds is valued
