Job Description

We are seeking a dedicated Family Practice Nurse Practitioner for an 80-day assignment in Maine, commencing Feb 1, 2027. This role involves working 4 10-hour shifts, providing full scope primary care, chronic care management, and supporting walk-in clinic needs, with a significant focus on a geriatric patient population and comfortable management of chronic medications.

Responsibilities and Duties

• Provide comprehensive family medicine care in a walk-in setting.
• Manage a patient panel with a focus on geriatric and older populations.
• Deliver full scope primary care services.
• Conduct chronic care management and medication management.
• Offer walk-in clinic support, including minor urgent care cases.
• Comfortably manage patients on chronic medications.
• Utilize onsite lab and X-ray services.
• Handle post-discharge follow-ups from the Emergency Department.
• Address higher acuity cases, typical for an older patient demographic.

Additional Information

• Board Certification in Family Medicine is required.
