Job Description

We are seeking a dedicated Family Practice Nurse Practitioner for an 80-day assignment in Maine, commencing Feb 22, 2027. This role involves working four 10-hour shifts, providing comprehensive primary care services. The successful candidate will manage chronic conditions, perform medication management, and support a walk-in clinic, primarily serving an older patient demographic.

Responsibilities and Duties

• Provide full scope primary care services.
• Manage chronic care conditions for patients.
• Oversee medication management protocols.
• Offer support within a walk-in clinic setting, including minor urgent care cases.
• Deliver care to a patient population consisting predominantly of geriatric and older individuals.
• Conduct post-discharge follow-ups for patients released from the ED.

• Required Board Certifications: Family Medicine
• EMR: Meditech EHR
• Comfort with managing patients on chronic medications is essential.
