Job Description

We are seeking an Internal Medicine Nurse Practitioner for a 120-day assignment starting May 7, 2026, in Tennessee. This role involves making home health visits within apartment-style subsidized housing, working 8a-5p shifts. You will conduct comprehensive anchor visits to establish care plans and perform follow-up appointments, managing acute care needs and ensuring continuous patient support.


Responsibilities and Duties

  • Perform home health visits within apartment-style subsidized housing, with an average travel radius of 30 minutes to 1-2 apartment buildings per day.
  • Conduct 90-minute comprehensive anchor visits to build full patient care plans.
  • Manage typically 30-minute follow-up appointments.
  • Provide acute visits and follow-ups as key responsibilities.
  • Dedicate the first hour of each day to administrative tasks and documentation.
  • Ensure all charts are closed within two business days.
  • For patient no-shows, utilize the time for documentation or attempt to fill the slot with another patient.
  • Adhere to no-show and outreach protocols, including door knocking, immediate patient calls, checking common areas, and coordinating with property staff to locate patients or fill the slot.
