Job Description

Our client in California is seeking a dedicated Family Practice Nurse Practitioner for an impactful 120-day assignment starting Aug 10, 2026. This role involves managing a diverse patient panel, including chronic disease, mental health, and LGBTQ+ focused care, with shifts from 8:00am – 5:00pm or 10:00am – 7:00pm. You will primarily focus on patient medication refills and a caseload of approximately 18 patients daily.


Responsibilities and Duties

  • See approximately 18 patients per day
  • Manage patient medication refills
  • Manage chronic diseases such as hypertension, type 2 diabetes, and asthma
  • Conduct annual wellness exams, employment physicals, and follow-up visits for chronic lab monitoring
  • Manage basic SSRI/SNRI titration for anxiety and depression, with referrals for specialized therapy
  • Perform screening for HIV/STI (PrEP & PEP)
  • Provide care for adolescent and adult trans or non-binary patients
  • Initiate and maintain hormone therapies, including Testosterone (IM/SubQ/Gel) or Estrogen with anti-androgens (Spironolactone)
  • Monitor hormone levels, including total/free Testosterone, Estradiol, CBC, and BMP
