Job Description

Our client in Pennsylvania is seeking a Family Practice Physician for a 60-day assignment starting Jul 13, 2026. The schedule involves working from 8:30 am to 5:00 pm, providing in-home primary care to a panel of adult and geriatric patients. This role focuses on conducting comprehensive evaluations, managing chronic and acute conditions, performing medication management, and coordinating care to ensure optimal patient outcomes within their homes.


Responsibilities and Duties

  • Provide in-home primary care to a panel of adult and geriatric patients
  • Conduct comprehensive initial evaluations and routine follow-up visits
  • Diagnose, treat, and manage chronic and acute conditions
  • Perform medication management, reconciliation, and adjustments
  • Develop and implement individualized care plans
  • Coordinate care with Nurse Practitioners/Physician Assistants, nurses, social workers, and care coordinators
  • Order and interpret labs, imaging, and diagnostic tests
  • Deliver preventative care, including screenings and wellness visits
  • Manage transitions of care, including hospital and Skilled Nursing Facility discharges
  • Communicate with patients, families, and caregivers regarding care plans
  • Address urgent patient needs and triage issues during assigned coverage periods


Additional Information

  • Required Board Certifications: Emergency Medicine, Family Medicine, Internal Medicine
  • EMR System: Athenahealth
  • Patient caseload includes management of chronic diseases such as diabetes, hypertension, congestive heart failure, and COPD, as well as geriatric care, post-hospital discharge follow-ups, and preventative wellness visits. Visits typically range from 30 to 60 minutes.
