Job Description

Our client in Rhode Island is seeking a dedicated Internal Medicine Physician for a 240-day assignment starting Aug 10, 2026. This role involves managing post-discharge transition and stabilization cases, focusing on medication reconciliation, addressing emerging patient risks, and overseeing post-operative care within a 9:00am to 5:00pm schedule. You will support patients navigating complex health challenges and social determinants of health to prevent readmissions.


Responsibilities and Duties

  • Manage post-discharge transition and stabilization cases, focusing on the critical 30-day post-hospitalization window.
  • Conduct exhaustive virtual medication reconciliation for polypharmacy patients to prevent adverse drug events.
  • Perform urgent, same-day virtual visits for emerging risks indicated by home biometrics, such as sudden weight gain or O2 saturation drops.
  • Oversee and manage patients following post-operative care from a hospital.
  • Handle complex cases including Advanced Heart Failure requiring remote fluid status tracking and diuretic adjustments.
  • Manage severe COPD and chronic lung disease patients post-exacerbation, focusing on oxygenation monitoring and inhaler therapy optimization.
  • Address brittle and uncontrolled diabetes cases with severe glycemic volatility and transition to intensive insulin/CGM regimens.
  • Navigate severe hypertension and CKD overlap, carefully titrating medications while considering kidney function.
  • Support patients with comorbid depression and anxiety that impact their ability to self-manage physical diseases.
  • Act as the clinical lead in cases threatened by severe Social Determinants of Health, directing support staff to address resource deficits, food insecurity, and medication access issues.
