Job Description

We are seeking a dedicated Geriatrics Nurse Practitioner for a 120-day assignment in North Carolina, starting Jun 15, 2026. This full-time role operates Monday through Friday, 8am-5pm, focusing on providing comprehensive medical, health, and social services within a PACE clinic for the geriatric population, including frail or elderly individuals aged 55 years and older.

Responsibilities and Duties

• Provide comprehensive medical, health, and social services to elderly participants in a PACE clinic setting.
• Work with frail or elderly individuals aged 55 years and older.
• Deliver adult day health programs and rehabilitation therapy.
• Care for patients aged 55+ who meet nursing home care requirements but reside safely in the community.
