Job Description
We are seeking a dedicated Family Practice Nurse Practitioner for a 60-day assignment in California, starting May 4, 2026. This role involves 8a-5p shifts and focuses on providing comprehensive care, including routine physical exams, managing acute and chronic conditions, prescribing medications, and overseeing geriatric and post-acute rehabilitation care. The successful candidate will collaborate closely with interdisciplinary teams to develop individualized care plans and educate residents and families on treatment and preventative care.
Responsibilities and Duties
- Perform routine physical exams, assess acute and chronic conditions, and manage ongoing treatment plans
- Order and interpret labs/diagnostics; prescribe medications and perform minor procedures
- Manage admissions, transfers, and discharges with accurate and timely documentation
- Collaborate with physicians, nursing staff, and therapy teams on individualized care plans
- Educate residents and families on treatment plans, medications, and preventative care
- Support quality improvement initiatives and ensure compliance with facility standards
- Manage chronic disease conditions such as diabetes, hypertension, COPD, and CHF
- Provide post-acute and rehab care following hospitalization or surgery
- Deliver geriatric care and manage complex comorbidities
- Oversee medication management and polypharmacy
- Perform minor bedside procedures, including wound care, depending on scope of practice
- Address functional decline, mobility issues, and activities of daily living (ADL)-related care planning
