Job Description

Our client in California is seeking a dedicated Wound Care Nurse Practitioner for a 5-day assignment starting Mar 30, 2026. This role involves comprehensive wound care assessments and treatments through home visits, operating within a mobile provider model. You will engage in independent clinical practice while collaborating closely with various care teams, with a minimum of 5 hours of pay guaranteed per day.


Responsibilities and Duties

  • Provide home visits within a mobile provider model across skilled nursing facilities, long-term care facilities, and post-acute rehab centers.
  • Work independently in clinical practice while collaborating with care teams.
  • Collaborate closely with Directors of Nursing, facility nurses, attending physicians, interdisciplinary care teams, and rehab staff.
  • Perform comprehensive wound assessments, including staging (pressure injuries stages 1–4, unstageable, deep tissue injuries), measurements, and etiology identification.
  • Document progression with photographic evidence.
  • Evaluate skin integrity and identify signs of infection, necrosis, or delayed healing.
  • Treat various wound types, including pressure ulcers, diabetic foot ulcers, venous stasis ulcers, arterial ulcers, surgical wounds, traumatic wounds, skin tears, infected wounds, and non-healing chronic wounds.
  • Manage patients with complex conditions such as diabetes, peripheral vascular disease, immobility, neuropathy, malnutrition, and chronic illness.
