Job Description
Our client in California is seeking a dedicated Family Practice Nurse Practitioner for a 40-day assignment, commencing May 25, 2026. This full-time role, Monday through Friday from 8:30 AM to 5:30 PM, involves assessing and diagnosing patients with multiple comorbidities, conducting wellness exams, and delivering comprehensive outpatient primary care within an integrated care team setting.
Responsibilities and Duties
- Assess and diagnose patients experiencing multiple comorbidities and conduct wellness exams and check-ups.
- Deliver comprehensive outpatient care across the primary care scope, including acute, chronic, and preventive services.
- Manage patient panels, with a particular focus on underserved and diverse populations.
- Diagnose, treat, and develop care plans for a wide range of conditions, such as diabetes, hypertension, and COPD.
- Perform routine procedures, including exams, minor office procedures, and basic diagnostics.
- Address standard primary care needs, including walk-ins for common ailments like cold, cough, and flu.
- Collaborate effectively as part of an integrated care team.
- Utilize translator applications to ensure effective communication with non-English speaking patients.
- Demonstrate empathy and patience, especially when working with underserved communities.
Additional Information
- Board Certified in Family Medicine or Family Nurse Practitioner (FNP).
- EMR System: Epic.
- Prior experience in an FQHC or IHS setting is highly valued.
- Proficiency in Women's Health is a significant advantage.
