Job Description

Our client in Maryland is actively seeking a dedicated Family Practice Nurse Practitioner for a 40-day assignment, commencing May 6, 2026. This vital role involves traveling to patient homes to conduct comprehensive home health assessments for Medicaid and Medicare patients. The ideal candidate will manage 8a-8p shifts, with schedules conveniently provided up to three weeks in advance, ensuring predictable work-life balance while delivering essential care.


Responsibilities and Duties

  • Conduct home health assessments for Medicaid and Medicare patients in their homes.
  • Utilize provided medical kits for all necessary equipment during home visits.
  • Complete all charting accurately within a 48-hour timeframe and call ahead for patient introductions.
  • Collaborate with a dedicated team for the processing and fulfillment of all lab orders.
  • Manage annual wellness visits and health risk assessments.
  • Perform basic diagnostic procedures, including A1C and retinal scans, and bone density where applicable.
  • Please note, this role does not involve blood draws or prescribing medication.
