Job Description

Our client in California is seeking a dedicated Wound Care Nurse Practitioner for a 2-day assignment, beginning Jun 5, 2026. This role involves traveling between skilled nursing, long-term care, and post-acute rehab facilities to deliver comprehensive wound assessments and management. Clinicians in this mobile provider model will perform a range of duties, with a minimum of 5 hours of work guaranteed per day.


Responsibilities and Duties

  • Provide care within Skilled Nursing Facilities (SNFs), long-term care, and post-acute rehab centers.
  • Operate within a mobile provider model, traveling between facilities.
  • Conduct comprehensive wound assessments, including staging (pressure injuries stages 1–4, unstageable, deep tissue injuries), measurements, and etiology identification.
  • Perform photographic documentation to track wound progression and evaluate skin integrity.
  • Identify and manage infection, necrosis, or delayed healing.
  • Treat a variety of wound types, such as pressure ulcers, diabetic foot ulcers, venous and arterial ulcers, surgical wounds, traumatic wounds, skin tears, and non-healing chronic wounds.
  • Collaborate closely with Directors of Nursing (DONs), facility nurses, attending physicians, interdisciplinary care teams, and rehab staff.
  • Maintain independent clinical practice with collaborative support.
