Responsibilities and Duties
Manage follow-up visits, including 45-minute sessions for medication adjustments, symptom monitoring, and chronic care management.
Order and interpret diagnostic tests such as EKGs, echocardiograms, stress tests, and lab tests.
Provide comprehensive patient education focused on lifestyle counseling, risk factor modification, and medication compliance.
Coordinate care by collaborating with cardiologists, primary care physicians, and other specialists to create comprehensive treatment plans.
Triage acute issues, assessing symptoms like chest pain or palpitations and directing urgent care when necessary.
Additional Information
Advanced Cardiac Life Support (ACLS) certification required.
Must be licensed to practice as a Nurse Practitioner in Oregon.
Familiarity with Meditech EHR is essential for managing patient information efficiently.
