Responsibilities and Duties
Function independently to deliver high-quality, patient-centric care in collaboration with the healthcare team
Provide care in hospital, clinic, and through telehealth modalities (telephone, video, secure email) as appropriate
Gather patient and collateral history, perform physical exams, and review prior records and ancillary data
Develop, implement, and monitor treatment plans utilizing current medical knowledge and sound judgment
Communicate effectively with patients, families, and the healthcare team to ensure coordinated care
Provide patient and family education and counseling to support treatment and discharge plans
Collaborate with physicians, leadership, and medical staff for safe and compliant patient care
Document care in accordance with regulatory and care site standards
Assist collaborating physicians with patient management in the hospital setting, consistent with site policies and scope of practice
Float to other departments within competency as required by business needs
Maintain regular attendance for on-site work during scheduled business hours or shifts
May be placed on-call during scheduled shifts as needed
Additional Information
Active Colorado NP license required
Basic Life Support (BLS) certification required
Ability to work nights and/or weekends as required by the position
