 We are seeking a skilled Family Practice Nurse Practitioner for a locum tenens position at a Multi-Specialty Clinic in Connecticut. This 120-day assignment begins on 08/04/2025, with potential for extension. The ideal candidate will be comfortable providing comprehensive primary care and family practice services, including chronic disease management, well visits, and treatment of common illnesses. The work schedule is Monday through Friday, typically 8 AM to 5 PM, for a total of 40 hours per week. Responsibilities include managing conditions such as diabetes, hypertension, and high cholesterol, as well as conducting check-ups and addressing acute health concerns like coughs, colds, and flu.

Responsibilities and Duties:

• Provide comprehensive primary care and family practice services

• Manage chronic diseases, including diabetes, hypertension, and high cholesterol

• Conduct well visits and check-ups for patients of all ages

• Diagnose and treat common illnesses such as coughs, colds, and flu

• Perform physical examinations and health screenings

• Develop and implement treatment plans

• Prescribe medications and therapies as appropriate

• Educate patients on preventive care and healthy lifestyle choices

• Collaborate with other healthcare professionals for patient care

• Utilize eClinicalWorks EMR for patient management and record-keeping

• Maintain a high standard of patient care and satisfaction

• Adhere to all clinic policies and procedures

• Maintain accurate and detailed patient records

Additional Information:

• Must be licensed to practice as a Nurse Practitioner in Connecticut

• Proficiency in eClinicalWorks EMR required

• Experience in family practice and primary care preferred

• Strong communication and interpersonal skills essential
