Responsibilities and Duties
Manage an independent patient panel with a focus on primary care and family practice
Provide care to a diverse patient population, including those with possible substance abuse, comorbidities, and dual diagnoses
See approximately 20 or more patients per day, managing 2-3 patients per hour
Demonstrate proficiency with EMR and timely chart completion
Prescribe and manage patients on pain medication as appropriate
Deliver care to underserved and community health populations
Additional Information
Connecticut state license required
Board certification in Family Medicine required
Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS) certifications required
Minimum of 2-3 years experience as a Nurse Practitioner preferred
Experience in community health settings strongly preferred
