Responsibilities and Duties

Work closely with the Vital Links clinical team to ensure proper patient follow-up and care coordination.
Additional Information
Must be licensed as a Nurse Practitioner in Illinois.
Experience in family practice and geriatric care preferred.
Strong interpersonal skills to effectively communicate with elderly patients and facility staff.
