Responsibilities and Duties
- Provide comprehensive care in Family Medicine and Behavioral Health
- Manage a 50/50 split of case loads between adults and children
- Collaborate with a team under the supervision of the Medical Director
- Utilize Smartcare EMR for maintaining patient records
- Ensure adherence to clinic protocols and quality standards
Additional Information
- Required Board Certifications: Family Medicine
- Experience: Minimum 2-3 years, with a preference for Behavioral Health experience
- Other Certifications: DEA certification required
- License: Valid IN nurse practitioner license
