Job Details

Responsibilities and Duties
  • Manage medications for patients
  • Educate patients on home treatment plans
  • Refer patients to appropriate specialists as needed
  • Ensure continuity of care between visits
  • Complete charting and notations for each patient consult in the EMR
  • Participate in peer reviews
  • Work collaboratively with the Chief Medical Officer and medical team
  • Provide care to a culturally and economically diverse patient population

Additional Information
  • Must be licensed as a nurse practitioner in Indiana
  • Required board certification in Family Medicine
  • Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR) certifications required
  • Comfort seeing patients of all ages
  • Experience with EMR systems required
