Responsibilities and Duties
Manage medications for patients
Educate patients on home treatment plans
Refer patients to appropriate specialists as needed
Ensure continuity of care between visits
Complete charting and notations for each patient consult in the EMR
Participate in peer reviews
Work collaboratively with the Chief Medical Officer and medical team
Provide care to a culturally and economically diverse patient population
Additional Information
Must be licensed as a nurse practitioner in Indiana
Required board certification in Family Medicine
Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR) certifications required
Comfort seeing patients of all ages
Experience with EMR systems required
