We are seeking a dedicated Family Nurse Practitioner for a 44-day locum tenens assignment in New York, starting on Aug 18, 2025. This role involves day shifts from Monday to Friday, focusing on primary care, chronic disease management, and follow-up care for acute visits and hospital stays. The successful candidate will be part of a supportive team, including a dedicated MA or LPN, managing the care of 15-18 patients per day.

  • Schedule: Monday - Friday, 8 AM - 4:30 PM; Call required after 2 months (phone call only, averaging 5-10 calls per week)

Responsibilities and Duties
  • Provide general primary care, manage chronic diseases, and handle acute visits along with ED and hospital follow-ups.
  • See an average of 15-18 patients per day.
  • Work collaboratively with a physician and a dedicated support staff member (MA or LPN).
  • Utilize Epic and Medent EMR systems to document and manage patient care.

Additional Information
  • Must be board certified or board eligible.
  • Must hold a valid New York license and be Medicaid NY enrolled.
  • Prior clinic experience is required.
  • Must be able to commit to a minimum of 6 months.
