The ideal candidate will work Monday through Friday, 8:00 AM to 5:00 PM, in multiple assisted living facilities. The role involves providing acute and chronic care management, developing treatment plans, and monitoring for early changes in conditions for contracted patients in these facilities. The schedule includes rotating between different locations throughout the week, with varying patient visit frequencies based on individual care plans.

Responsibilities and Duties:
• Provide acute and chronic care management for contracted patients in assisted living facilities
• Develop and implement treatment plans
• Monitor patients for early changes in conditions
• Conduct patient assessments and examinations
• Prescribe medications and treatments as needed
• Collaborate with facility staff and other healthcare professionals
• Maintain accurate and detailed patient records using GEHRIMED and PointClickCare systems
• Adhere to all facility protocols and safety standards
• Participate in patient and family education
• Manage follow-up care and referrals as needed
• Adapt to varying patient visit frequencies (monthly, quarterly, annually, or acute visits only)
Additional Information:
• Must be licensed to practice as a Nurse Practitioner in West Virginia
• Experience in geriatric or long-term care settings is preferred
• Proficiency in GEHRIMED and PointClickCare systems is beneficial
• Flexibility to work at multiple locations throughout the week
