This full-time opportunity involves caring for patients of all ages and follows a schedule of day shifts: 8am-5pm on Monday, Wednesday, and Friday, and 10am-7pm on Tuesday and Thursday.

Responsibilities and Duties
  • Provide comprehensive medication management
  • Educate patients on home treatment protocols
  • Refer patients to appropriate specialists as needed
  • Ensure continuity of care between patient visits
  • Complete accurate charting and notes in the EMR system for each consult
  • Participate in peer reviews
  • Collaborate with the Chief Medical Officer and broader medical team
  • Deliver care to a culturally and economically diverse patient population

Additional Information
  • Must hold an active Indiana license and be board certified in Family Medicine
  • Required certifications: Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR)
  • Experience working with patients of all ages is necessary
