Job Description

Our client in California is seeking a dedicated Geriatrics Physician for an 80-day assignment starting Mar 23, 2026. This role involves working 8a-5p, primarily within a PACE Program, providing comprehensive care for highly complex geriatric patients, including rounding at skilled nursing facilities and managing chronic conditions.


Responsibilities and Duties

  • Provide comprehensive care within a PACE Program, managing a mix of PACE and Skilled Nursing Facility (SNF) patient needs.
  • Conduct low-volume rounds on SNF patients, focusing on care plans for timely discharge.
  • Collaborate with an interdisciplinary care team from 8a-5p to ensure timely discharge for highly complex patients.
  • Manage complex cases, including reviewing social determinants of health and coordinating home care.
  • Address conditions such as dementia, Alzheimer's, and other geriatric health issues.
  • Oversee geriatric patients on 15-20+ medications, handling regular follow-ups, acute care cases, chronic care, and comorbidities.


Additional Information

  • Requirement: Must be comfortable rounding at 2-3 skilled nursing facilities within a 30-minute radius.
