This opportunity offers weekday, day shifts (Monday to Friday, 8am–5pm) focused on outpatient care for geriatric patients, emphasizing chronic care management and preventative medicine.

Responsibilities and Duties
  • Provide comprehensive outpatient care to geriatric patients
  • Focus on chronic care management and preventative medicine to minimize hospital admissions
  • Accurately code and chart patient encounters
  • Utilize technology efficiently to support clinical workflows
  • Maintain organized records and support continuity of care

Additional Information
  • Active Florida medical license required
  • Board certification in Family Medicine, Geriatric Medicine, or Internal Medicine required
  • Basic Life Support (BLS) certification required
  • 2-3 years of post-residency experience, including work with chronic care cases in community health centers
  • Strong organizational and technology skills are essential
