Responsibilities and Duties
Deliver attentive, patient-centered care for geriatric patients with chronic and preventative needs
Accurately code and chart patient encounters, maintaining organized, thorough documentation
Utilize eClinicalWorks EMR for all patient records and communications
Collaborate with the healthcare team to coordinate care and minimize hospital admissions
Serve as a resource for chronic care management within the community health center
Apply technology efficiently in daily workflows to optimize patient care
Uphold the highest standards of safety and quality in all clinical activities
Additional Information
2-3 years post-residency experience preferred, specifically in community health centers focused on chronic care
Board certification required in Family Medicine, Geriatric Medicine, or Internal Medicine
Active Missouri license required
Basic Life Support (BLS) certification required
Experience in geriatrics and preventative medicine strongly preferred
Candidates should be highly organized and tech savvy
