Responsibilities and Duties
Provide primary medical care to adult patients, ranging from 19 to 70s, in a long-term care setting
Manage a patient census with a focus on low acuity, including those with dementia, distal muscular dystrophy, and traumatic brain injury
Conduct routine assessments and ongoing management of chronic and acute medical conditions
Collaborate with interdisciplinary teams to develop care plans and support patient needs
Document patient care thoroughly in compliance with facility protocols and standards
Additional Information
Rhode Island Nurse Practitioner license required
Board certification in Family Medicine or Internal Medicine required
Experience with adult patients in post-acute, long-term, or skilled nursing settings is preferred
No ICU responsibilities
