  • Conduct routine assessments and ongoing management of chronic and acute medical conditions
  • Collaborate with interdisciplinary teams to develop care plans and support patient needs
  • Document patient care thoroughly in compliance with facility protocols and standards

Additional Information
  • Rhode Island Nurse Practitioner license required
  • Board certification in Family Medicine or Internal Medicine required
  • Experience with adult patients in post-acute, long-term, or skilled nursing settings is preferred
  • No ICU responsibilities
