Job Description

Our client in NY is seeking an Internal Medicine Nurse Practitioner for a 36-day assignment beginning Mar 2, 2026. This role involves conducting home visits for at-home assessments and annual wellness exams, with a daily schedule from 8:30 AM to 4:30 PM, offering some flexibility. The provider will be responsible for comprehensive charting and documentation for each patient interaction.


Responsibilities and Duties

  • Conduct home visits for at-home assessments and annual wellness exams.
  • Travel to elderly patients' homes for appointments.
  • Work independently on assignments.
  • Complete extensive charts and documentation for each visit within 48 hours.
  • Be prepared for chart audits and complete them as required.
  • Utilize provided PPE during visits.
  • Maintain awareness of patient health statuses, including potential HIV positive cases.
  • Communicate with the client representative if uncomfortable during a visit to reschedule with an escort.
  • Provide own vehicle for travel.
