Responsibilities and Duties
Deliver basic medical services to adult patients aged 19 to 70s
Manage a patient panel primarily at nursing home level acuity, including individuals with dementia, distal muscular dystrophy, and traumatic brain injury
Provide care in a facility licensed as an LTACH but functioning as a standard long-term care or skilled nursing facility
Collaborate with interdisciplinary teams to ensure optimal patient outcomes
Maintain accurate and timely documentation of patient care
Adhere to all facility protocols and care standards
Additional Information
Rhode Island license required
Board certification in Family Medicine or Internal Medicine required
Experience with long-term care or similar patient populations preferred
