Locum Tenens Nurse Practitioner - Internal Medicine Needed in TN

Job Description

We are seeking an Internal Medicine Nurse Practitioner for a 60-day assignment in Tennessee, starting Apr 13, 2026. This role involves home health visits within apartment-style subsidized housing, with a typical schedule of 8a-5p and a travel radius of roughly 30 minutes to 1-2 apartment buildings daily. Responsibilities include 90-minute comprehensive initial visits and 30-minute follow-up/acute visits.

Responsibilities and Duties

• Conduct home health visits within apartment-style subsidized housing.
• Travel with a radius of approximately 30 minutes, expecting to visit 1-2 apartment buildings daily.
• Perform 90-minute comprehensive anchor visits to establish full care plans.
• Manage acute visits and follow-up appointments, typically 30 minutes in duration, under a fee-for-service model.
• Allocate the first hour of each day for administrative tasks and documentation.
• Ensure all charts are closed within 2 business days.
• Adhere to a no-show and outreach protocol: initiate a door knock, wait 5 minutes, call the patient immediately if there is no answer, check common/public areas in the building, and coordinate with property staff.
• If a patient remains unavailable, review charts, attempt to see other patients in the building, and coordinate with field staff to fill the time slot.
