This opportunity offers a flexible Monday through Thursday schedule, with options of either 8:00 AM to 4:00 PM or 9:00 AM to 5:00 PM, focusing on adult primary and chronic care management.

Responsibilities and Duties

• Conduct wellness checks and provide preventative care for adult patients
• Manage chronic conditions such as hypertension, diabetes, and hyperlipidemia
• Perform acute sick visits including evaluation and treatment of cold, flu, and infections
• Oversee medication management and prescription refills
• Interpret laboratory results and communicate findings to patients
• Perform minor office procedures as required
