Our client in Utah is seeking a dedicated Preventive Medicine Nurse Practitioner for a 60-day assignment starting May 25, 2026. This role involves providing essential preventative care to a large homeless population and general super utilizers of healthcare services, with a strong emphasis on reducing hospitalizations through regular patient contact during weekday shifts from 9:30 AM to 5:30 PM.

Responsibilities and Duties

• Provide preventative care to a large homeless population and general super utilizers of healthcare services.
• Make regular contact with all patients, typically every couple of weeks, with a strong focus on reducing hospitalizations.
• Manage chronic care cases, including diabetes, heart disease, and complex case management.
