Our client in Utah is seeking a Preventive Medicine Physician Assistant to begin a 60-day assignment on May 25, 2026. This role involves daily shifts from 9:30 AM to 5:30 PM, focusing on providing preventative care to a large homeless population and frequent healthcare users. The Physician Assistant will maintain regular patient contact every couple of weeks, with a strong emphasis on reducing hospitalizations.

Responsibilities and Duties

• Provide preventative care on a large homeless population and general super utilizers of healthcare services.
• Make contact with all patients every couple of weeks.
• Focus on reducing hospitalizations.
• Manage chronic care (diabetes, heart disease, general super utilizers of healthcare services) and complex case management.
