Job Description

We are seeking a dedicated Wound Care Nurse Practitioner for a 4-day assignment in California, commencing Apr 3, 2026. This role involves a mobile provider model focusing on home visits and travel between skilled nursing, long-term care, and post-acute rehab facilities. The schedule guarantees a minimum of 5 hours of pay per day, where you will perform comprehensive wound assessments and manage various complex wound types for patients with diverse chronic conditions.


Responsibilities and Duties

  • Perform comprehensive wound assessments including staging (pressure injuries stages 1–4, unstageable, deep tissue injuries), measurements (length, width, depth), and etiology identification.
  • Conduct photographic documentation for tracking wound progression.
  • Evaluate skin integrity and identify signs of infection, necrosis, or delayed healing.
  • Treat typical wound types such as pressure ulcers, diabetic foot ulcers, venous stasis ulcers, arterial ulcers, surgical wounds, traumatic wounds, skin tears, and infected chronic wounds.
  • Collaborate closely with Directors of Nursing, facility nurses, attending physicians, interdisciplinary care teams, and rehab staff.
  • Practice independently within a care team collaboration model.
