Job Description

Our client is actively seeking a dedicated Wound Care Nurse Practitioner for a 4-day locum tenens assignment in California, commencing Apr 6, 2026. In this vital role, you will provide comprehensive wound assessments and care within a mobile provider model, performing crucial home visits and working a minimum of 5 hours per day. This is an excellent opportunity to deliver specialized patient care within a supportive environment.


Responsibilities and Duties

  • Perform comprehensive wound assessments.
  • Conduct wound staging (pressure injuries stages 1–4, unstageable, deep tissue injuries).
  • Perform wound measurements (length, width, depth).
  • Identify wound etiology.
  • Utilize photographic documentation for tracking progression.
  • Evaluate skin integrity.
  • Identify infection, necrosis, or delayed healing.
  • Treat a variety of wound types including pressure ulcers, diabetic foot ulcers, venous stasis ulcers, arterial ulcers, surgical wounds, traumatic wounds, skin tears, infected wounds, and non-healing chronic wounds.
  • Care for patients with common co-morbidities such as diabetes, peripheral vascular disease, immobility, neuropathy, malnutrition, and chronic illness.
  • Work within skilled nursing facilities (SNFs), long-term care facilities, and post-acute rehab centers.
  • Operate within a mobile provider model, traveling between facilities.
  • Practice independent clinical practice with collaborative support.
  • Collaborate closely with Directors of Nursing (DONs), facility nurses, attending physicians, interdisciplinary care teams, and rehab staff.
