Provider Type Needed: Wound Care Nurse Practitioner
Schedule: Flexible day shifts between 8am-5pm, Monday through Friday; mutually agreed weekly schedule with the client; visit length varies by patient need
Responsibilities and Duties
Conduct comprehensive wound assessments and develop individualized care plans
Perform wound care procedures such as debridement, advanced dressing applications, negative pressure wound therapy, and MIST therapy
Document all care accurately and completely in the electronic medical record system in accordance with Medicare and company requirements
Educate patients, families, and caregivers on wound care management, prevention, and follow-up care
Collaborate with physicians, facility staff, and care teams to ensure coordinated patient care
Monitor patient progress and adjust treatment plans as needed
Maintain compliance with all applicable federal and state laws, including Medicare documentation and billing standards
Participate in periodic training, audits, and quality assurance reviews
Additional Information
Must be licensed as a Nurse Practitioner in Illinois
Wound Care Certification required
Independent work with support from clinical and administrative teams
Flexible, mobile care model visiting patient homes, assisted living, and residential care settings
Part-time role with potential to increase hours based on provider fit and interest
