Responsibilities and Duties
Provide advanced wound care treatment for wounds not healed under conservative care for over 30 days
Conduct initial assessments and develop individualized care plans
Apply amniotic membrane grafts as the primary treatment modality
Utilize amniography in wound management
Place orders for synthetic skin or skin grafts as clinically indicated
Perform debridement and rewrapping of wounds
Travel to patients' homes for one-on-one care visits
Maintain detailed and accurate clinical documentation
Additional Information
Active Indiana nurse practitioner license required
Board certification in Wound Care required
Advanced wound care experience preferred
Ability to work independently and confidently in home-based care settings
