Responsibilities and Duties
Deliver direct wound care services to patients, primarily in long-term care facilities and residential settings
Utilize advanced wound care modalities, including UltraMist Wands and application of skin grafts (fish skin, placenta, lab-grown)
Travel to patient residences and long-term care sites to provide clinical wound care
Collaborate with facility staff to ensure continuity and quality of wound care
Maintain accurate documentation in the WoundExpert EMR system
Apply evidence-based practices and adhere to clinical protocols
Additional Information
Must have active Nurse Practitioner license in Minnesota
Wound, Ostomy and Continence Nursing (WOCN) certification required
Prior hands-on wound care experience is essential
Experience with surgical PAs and podiatry is preferred
Experience with UltraMist Wands and various skin grafting techniques highly desired
Providers with advanced wound care backgrounds will be prioritized
