Job Description

We are seeking a dedicated Wound Care Physician Assistant for our client in California for a 5-day assignment beginning Mar 30, 2026. This role focuses on delivering comprehensive home visit-based advanced wound care assessments and management across skilled nursing facilities, long-term care, and post-acute rehab centers. The schedule guarantees a minimum of 5 hours of pay per day, offering flexibility in patient care delivery.


Responsibilities and Duties

  • Conduct home visits within skilled nursing facilities, long-term care facilities, and post-acute rehab centers.
  • Operate within a mobile provider model, traveling between facilities.
  • Practice independently with robust clinical collaboration.
  • Collaborate closely with Directors of Nursing, facility nurses, attending physicians, interdisciplinary care teams, and rehab staff.
  • Perform comprehensive wound assessments, including staging, measurements, and etiology identification.
  • Document wound progression through photographic records.
  • Evaluate skin integrity and identify signs of infection, necrosis, or delayed healing.
  • Manage and treat a wide range of wound types, including pressure ulcers, diabetic foot ulcers, venous and arterial ulcers, surgical wounds, traumatic wounds, skin tears, and infected chronic wounds.
  • Care for patients presenting with complex conditions such as diabetes, peripheral vascular disease, immobility, neuropathy, malnutrition, and chronic illness.
