Responsibilities and Duties:
• Provide comprehensive primary care services to patients of all ages
• Manage chronic conditions and perform medication management
• Handle occasional urgent care cases (minimal suturing required)
• Conduct patient examinations, diagnose conditions, and develop treatment plans
• Order and interpret diagnostic tests
• Prescribe medications and provide patient education
• Collaborate with other healthcare professionals to ensure optimal patient care
• Maintain accurate and detailed medical records
• Adhere to FQHC policies and procedures
• Participate in quality improvement initiatives
Additional Information:
• West Virginia Nurse Practitioner license required for this position
• Experience in managing chronic conditions is essential
