A Private Practice in California is seeking a skilled Geriatrics Physician for a locum tenens assignment in a PACE (Program of All-Inclusive Care for the Elderly) program. This 11-day opportunity begins on June 19, 2025, with shifts running Monday through Friday from 8 AM to 5 PM.

Job Responsibilities and Duties:

- Provide comprehensive geriatric care in a PACE program setting

- Manage a patient census of 56 participants (not all seen daily)

- Handle regular follow-ups, acute care cases, and chronic care management

- Address comorbidities and complex cases in geriatric patients

- Participate in Interdisciplinary Team (IDT) meetings

- Review and consider social determinants of health

- Evaluate and manage home care needs

- Treat patients with conditions such as dementia and Alzheimer's disease

- Utilize Epic EMR for patient management and record-keeping

Additional Information:

- Experience in geriatric medicine and PACE programs preferred

- Strong understanding of complex care management for elderly patients

- Comfort with interdisciplinary team approach

- Ability to consider holistic patient needs, including social determinants of health

- Excellent communication skills for patient and family interactions

- Cultural competence and patient-centered care approach essential
